29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      (a) On 02/22/2017 - at Building #2281 A-P, 2891 Acton Rd, Birmingham, AL 35243, eye protection was not used when employees were operating a Hitachi NV 45AB2 1 3/4" Coil Nailer and Rolair System Air Compressor to nail shingles, exposing employees to flying particles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) On 02/22/2017 - at Building #2281 A-P, 2891 Acton Rd, Birmingham, AL 35243, employees were exposed to fall hazards of approximately 19 feet above ground level while removing old shingles and roofing felt and installing new roofing felt and shingles.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On 02/22/2017 - at Building #2281 A-P, 2891 Acton Rd, Birmingham, AL 35243, a fiberglass extension ladder used to access the roof was not extended at least 3-feet above the upper landing surface, was not secured to a rigid support, and/or a grasping device was not provided.
