29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about 2/23/17, inside building under construction at the West Buffalo Charter School, 2-Story Addition, 113 Lafayette Ave., Buffalo, NY: An HDX Model SFC1-500B, 20 inch High Velocity Floor Fan used for drying mud (joint compound for drywall) was in the corridor outside a classroom on the first floor. The fan had a metal frame and blade. The cord and plug on the fan was of the 2-wire type that had no grounding conductor and a 2 prong plug. The fan was not double insulated.     b) On or about 2/23/17, inside building under construction at the West Buffalo Charter School, 2-Story Addition, 113 Lafayette Ave., Buffalo, NY: A second fan of the same make and model (An HDX Model SFC1-500B, 20 inch High Velocity Floor Fan) was on the second floor, and also had a cord of the 2-wire type that had no grounding conductor and a 2 prong plug. It was connected to a 120 volt-ac receptacle via an extension cord.      NO ABATEMENT CERTIFICATION REQUIRED
