29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a) Facility, 1297 Kecksburg Road, Mount Pleasant, PA:  On or about February 23, 2017 the employer did not provide training to employees to ensure they understood the purpose and function of the energy control program.  Employees who service equipment had not been trained in the safe application, use and removal of control devices; employees working in and around servicing equipment had not been instructed about energy control.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Facility, 1297 Kecksburg Road, Mount Pleasant, PA:  On or about February 23, 2017 machine guarding was not provided on the Black Bros. laminating machine in that the opening between the rolls and the guard measured approximately 1 3/8 inches and did not prevent the operator from contact with the ingoing nip points while cleaning the rolls.

29 CFR 1910.213(c)(3): Handfed ripsaw(s) did not have non-kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator:    (a) Facility, 1297 Kecksburg Road, Mount Pleasant, PA:  On or about February 23, 2017 the Grizzly left tilting table saw being used to rip wood did not have non-kickback fingers to prevent the material from being thrown back towards the operator.
