29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At 13333 Lakefront Drive, Earth City, MO 63045, specifically inside of the Production Department, Room 18, the conveyor system belonging to the Bottle Unscrambler Machine, m/n AU-6C, s/n 3656, was inadequately guarded.  Employee(s) were exposed to the hazard of in-going nip points.  This violation was observed on 02/22/17.  At 13333 Lakefront Drive, Earth City, MO 63045, specifically inside of the Maintenance Department, the rotating parts belonging to the following machines were unguarded as of 02/22/17:  a/  JET Belt Sander/Disc Sander, m/n JSG-600, s/n 708599.  b/  Disc Sander.  c/  KBC Milling Machine, m/n TUM-1VS, s/n 122866.  d/  KBC Drilling Machine, m/n ZJ4132.  e/  KBC Lathe, m/n GRIP-1340G, s/n 220215.

29 CFR 1910.212(b):  Machines designed for a fixed location were not securely anchored to prevent walking or moving:  At 13333 Lake Front Drive, Earth City, MO 63045, specifically inside of the Maintenance Department, the JET Dual Abrasive Wheel Grinder, m/n JBG-8A, s/n 15010695, and the KBC Drilling Machine, m/n ZJ4132, were not anchored to the concrete floor or the equivalent.  This violation was observed on 02/22/17.

29 CFR 1910.219(d)(1):  Pulleys, any parts of which were seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section:  At 13333 Lake Front Drive, Earth City, MO 63045, specifically inside of the Maintenance Department, the horizontal pulley belonging to the KBC Milling Machine, m/n TUM-1VS, s/n 122866, was unguarded at two points.  The pulley was approximately six feet above the concrete floor.  Employee(s) were exposed to a caught-in hazard.  This violation was observed on 02/22/17.
