29 CFR 1910.147(d)(4)(i): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and was not done in the following sequence: Lockout or tagout device application. Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. At 13333 Lakefront DR, Earth City, Mo 63043, on or about March 22, 2023 employees were exposed to the hazards associated with the unexpected energization of equipment from no lock device applied to a Kaps All Capper Model AU-6C, S/N 3656, while an employee attempted to clear bottles from a lower compartment.
