29 CFR 1910.119(j)(2): The employer did not implement written procedures to maintain on-going integrity of process equipment as required.   On or about February 12, 2017, and at times prior to, in the Sulfur Recovery Unit at Houston Refining in Houston, TX, the employer did not ensure that the frequency of inspections and testing of process piping was performed in accordance with INP-PRO-201, Inspection, Testing, and Repair of Piping when:  a) The employer failed to inspect piping circuit 42-TG-1023-CAD (ISO439-0038-04), at the designated 5 year inspection interval (INP-PRO-201 Section 4.2.4.2.2 Table II) at the following locations:     1. TML 1     2. TML 2     3. TML 3     4. TML 4     5. TML 5     6. TML 6     7. TML 7     8. TML 8     9. TML 9     10. TML 10     11. TML 11     12. TML 12     13. TML 13     14. TML 14     15. TML 15     16. TML 16     17. TML 17   b) After the piping was placed in service, subsequent measurements of 42-TG-1023-CAD (ISO439-0038-04), were not made at appropriate intervals to establish corrosion (INP-PRO-201 Section 4.1.10), at the following locations:     1. 29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience. On or about February 12, 2017, and at times prior to, in the Sulfur Recovery Unit at Houston Refining in Houston, TX, the employer did not ensure that the frequency of inspections and testing of process piping was performed in accordance with Recognized and Generally Accepted Good Engineering practices such as but not limited to API RP 570 (Piping Inspection Code: In-service Inspection, Rating, Repair, and Alteration of Piping Systems), when: a) The employer failed to inspect piping circuit 42-TG-1023-CAD (ISO439-0038-04), at the designated 5 year inspection interval (API RP 570, Table 2) at the following locations: 1. TML 1 2. TML 2 3. TML 3 4. TML 4 5. TML 5 6. TML 6 7. TML 7 8. TML 8 9. TML 9 10. TML 10 11. TML 11 12. TML 12 13. TML 13 14. TML 14 15. TML 15 16. TML 16 17. On or about February 12, 2017 and at times prior to, in the Sulfur Recovery Unit at Houston Refining in Houston, TX, the employer operated piping circuit 42-TG-1023-CAD (ISO439-0038-04), with a wall thickness less than required for safe operation.    Houston Refining, LP was previously cited for a violation fo the Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.119(j)(5) which is contained in OSHA Inpsection Number 1139040, Citation Number 1 item 2, issued on September 15, 2016 with respect to a workplace located at 12000 Lawndale, Houston, TX.  This file became final order on February 29, 2017.
