29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.    1) Jobsite: Employees were using a Wal-Mart Mainstays Microwave to heat up food at lunch time that has had the grounding pin removed from the cable, thus exposing employees to an electrical hazard. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     1) Jobsite: The employer did not ensure that employees performing framing work on the roof of a new residential construction, approximately 25 feet from the ground below, wore fall protection. This condition exposed employees to fall hazards.     Condition noted on/or about 13 FEB 2017.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  1) Jobsite: The employer failed to provide adequate training for employees to recognize and mitigate fall hazards on the jobsite. Employees working on framing the roof of a new residential construction were working without fall protection approximately 25 feet above the ground below. Employees using defective/damaged electronics are exposed to shock hazards. Employees using a powered industrial truck without training prior to operation.  Condition noted on/or about 13 FEB 2017.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement.      1) Jobsite: Employer did not ensure the Werner fiberglass extension ladder, used to access the upper floors, was properly mounted on a stable and level surface to prevent accidental displacement thus exposing employees to a fall hazard. Employees used a piece of 2x4 board wedged under the tire of the powered industrial truck to stabilize the ladder which was missing its stabilizing feet.    Condition observed on/about 13 FEB 2017.
