29 CFR 1926.451(f)(6): The clearance between scaffolds and uninsulated power lines was not a minimum of 10 feet when scaffolds were erected, used, dismantled, altered, or moved such that they or any conductive material handled on them might come closer to exposed and energized power lines: a) 512 E Girard Ave, Philadelphia, Pa - The employer did not ensure that the erected fabricated frame scaffold, approximately 40 feet high, 49 feet long and 5 feet wide, had clearance greater than 10 feet from the 13.2kv energized uninsulated power lines thus exposing the employees to electrocution hazards. This condition was observed on or about February 23, 2017.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) 512 E Girard Ave, Philadelphia, PA - The employer did not ensure that guardrails were installed on the ends of the erected fabricated frame scaffold, approximately 40 feet high, 49 feet long and 5 feet wide, thus exposing the employees to fall hazards. This condition was observed on or about February 23, 2017.
