29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances were not designed for hard or extra-hard usage:    (a) New residential construction on Lot #33, Rear side: On or about February 24, 2017, and at times prior, the employer exposed employees to electrical shock hazards while using light duty interior type electrical cords, in that employees were not provided 3-wire heavy duty cords as they were submerged in standing ground water. A screw gun used to install metal roofing was plugged into this light duty cord set.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)  New residential construction on Lot #33, Rear roof: On or about February 24, 2017, and at times prior, the employer exposed employees to fall hazards of 13-feet, 1-inch from the lower ground level, in that employees were not provided with an adequate fall protection system while engaged in metal roofing activities. Employees were provided with full body harnesses with no connection to a life-line or anchor point.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder:    (a) New residential construction located on Lot #33, Rear side: On or about February 24, 2017, and at times prior, the employer allowed employees to descend from the roof without facing the ladder which exposed employees to falls up to 13-feet to the lower level, in that employees accessing the lower level did not face the ladder as they came down from the roof working surface.
