29 CFR 1910.28(b)(5)(i): Employees on a runway or similar walkway were not protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. a) "A" Platform, Plant 1, 72 East Market Street, Middleburg, PA - Employees worked on modular home components with setbacks, shortened widths, and/or vertical wall openings from an elevated catwalk and/or wooden flip-outs without a guardrail system exposing the employees to a fall hazard of approximately eight-foot (~8') onto a concrete floor, on or about February 20, 2017. b) "B" Platform, Plant 1, 72 East Market Street, Middleburg, PA - Employees worked on modular home components with vertical wall openings from an elevated catwalk and/or wooden filp-outs without a guardrail system exposing the employees to a fall hazard of approximately ten-foot (~10) onto a wooden floor on or about February 20, 2017.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: (A) Guardrail systems; (B) Safety net systems; or (C) Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) Plant 1, 72 East Market Street, Middleburg, PA - Employees worked on top of a modular home component without adequate protective equipment such as but not limited to guardrail systems, safety net systems, or personal fall protection systems exposing the employees to a fall hazard of thirteen-foot, ten-inches (13',10") onto a concrete floor, on or about February 20, 2017. In accordance with 29 CFR 1903.19(c), abatement certification and documentation is required for this violation. The Professional Building Systems, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1) or in the alternative 1910.132(a), which was contained in OSHA inspection number 1088963, citation number 1, item number 2 and was affirmed as a final order on December 1, 2015, with respect to a workplace located at 72 East Market Street, Middleburg, PA.
