29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a)  At the jobsite, on or about 2/10/17:  The employer did not protect employees using a Dynoflux Rod Oven that had the power cord pulled free of the box.  The cord insulation was pulled free of the box and the box held a current.
