29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: Grinding Dept., O.D. Grinders. On or about February 27, 2017 through March 14, 2017, employees who performed servicing and maintenance tasks on grinding machines, such as adjustment of the wheel diamond radius with a tool while the wheel was revolving, were not required to control hazardous energy sources in the following sequence: 1) turn off and shut down the machine, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy, 5) verify that isolation and de-energization of the machine or equipment has been accomplished. Employees were exposed to laceration and fracture hazards because alternate protections (such as machine guarding) were not adequate to prevent them from contacting the exposed sides of revolving grinding wheels. Abatement documentation is required in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Grinding Dept., O.D. Grinding Machines - On or about February 27 through March 14, 2017, operators were exposed to amputation and laceration hazards when manually placing and removing parts on/off of magnetic and air chuck collets. Operators' hands/fingers were in close proximity to the outer diameter of 20" grinding wheels revolving at 1200 rpm. There was no method of guarding utilized to prevent operators from contacting the moving grinding wheels. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
