OSH Act of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fires and explosions during acid and water flushing operations of natural gas wells:          a.  On or about February 16, 2017, and at times prior to, at Krebbs1-17H at the Davenport Well Pad, an employee was exposed to explosion and fire while opening a master valve to attempt to restart flow of a natural gas well which had deterioration in a gasket, flanges, and tubing.

29 CFR 1910.1200(e)(1)(ii): The written hazard communication program did not include the methods the employer would use to inform employees of the hazards of non-routine tasks, and the hazards associated with chemicals contained in unlabeled pipes in their work areas:       a. On or about February 16, 2017, at the Davenport Well Pad, the written hazard communication program did not include the methods the employer would use to inform employees of the hazards of non-routine tasks including, but not limited to flushing gas wells with Premox which exposes lease operator employees to hazardous chemicals.           b. On or about February 16, 2017, at the Davenport Well Pad, the written hazard communication program did not include the methods the employer would use to inform employees of the hazards associated with chemicals in unlabeled pipes in their work areas including, but not limited to high pressure gas (flammable) piped into gas wells which exposes lease operator employees to physical and chemical hazards.
