29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) Jobsite: On February 28, 2017 and at times prior thereto employees performed masonry preparation work (wiring and scratch) from a tubular welder scaffold. Employees were exposed to a failure of the scaffold system where mud sills were not used along with the base plates on all of the scaffold legs. The base plates rested on soft, muddy ground. 29 CFR 1926.454(b)(2): The employer did not have each employee who was involved with erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize the correct procedures for the work and type of scaffold in question:   a) Jobsite: On February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to fall hazards. Employees did not recognize or acknowledge the hazard of working without a complete guardrail system in place.  Actions were not taken to correct or minimize the hazard.    b) Jobsite: On February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to a potential failure of the scaffold.  Employees did not recognize or acknowledge the hazard of having multiple base plates without mud sills on soft, muddy ground.  Actions were not taken to correct or minimize the hazard.   c) Jobsite: On or about February 28, 2017 and at times prior thereto employees working from a tubular welded scaffold system were exposed to fall hazards where the working platforms were not fully planked.  Employees did not recognize or acknowledge the hazard and take appropriate actions to correct or minimize the hazard.     Note) Certification and documentation of corrective action ARE required for this item.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  a) Jobsite: On or about February 28, 2017 and at times prior thereto employees performed masonry preparation work (wiring and scratch) from a tubular welder scaffold. The employee(s) were exposed to a 19 foot fall hazard to the ground below where the top levels of the scaffold system were not fully decked.     H & A Plastering was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 468179, citation 1, item number 1a, and was affirmed as a final order on August 24, 2012, with respect to a workplace located at 3401 East Fairview Ave, Meridian, ID 83642.   Note) Certification of corrective action IS required for this item.

The employee(s) were exposed to a 19 foot fall hazard to the ground below where guardrail systems, including but not limited to, end rails were not in place.    H & A Plastering was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4), which was contained in OSHA inspection number 468179, citation 1, item number 1c, and was affirmed as a final order on August 24, 2012, with respect to a workplace located at 3401 East Fairview Ave, Meridian, ID 83642.   Note) Certification of corrective action IS required for this item.
