29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:   a) Job site, at/near 4106 Howley Street, Pittsburgh, PA: On or about February 23, 2016, during construction activities, the employer did not ensure that employees used a safe means of access to/egress from a scaffold in that they had a step ladder folded up and leaning against a building wall, then traversed from it to the scaffold outrigger and in the process exposed themselves to a fall of approximately 23 feet.  Richard Roth was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 908677, citation number 1, item 2b, and was affirmed as a final order on July 3, 2013, with respect to a workplace located at 318 45th Street, Pittsburgh, PA.
