29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: Smelter, gas cylinder storage area: Cylinders of oxygen and acetylene were not adequately separated by a fire wall or separated by 20 feet.

29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread: Smelter, first floor: Three instances of stairs with four or more risers were not equipped with standard railings. Specifically, the mid rails were missing the top railings did not extend the full length of the stairs creating tripping and falling hazards.
