29 CFR 1926.102(a)(1):  Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    Employees were exposed to a struck-by hazard. This was most recently documented on March 2, 2017 at 1136 W College Street Building 200 Liberty, MO 64068. An employee was observed using a pneumatic nail gun without the use of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Employees were exposed to a fall hazard. This was most recently documented on March 2, 2017 at 1136 W College Street Building 200 Liberty, MO 64068. Six employees were working on the roof of a three story apartment building removing and then replacing the shingles of the roof. Each of the employees had on a fall harness; none of the employees were tied off at any point on the roof. The employees were exposed to a fall hazard of approximately 22 feet with a roof pitch of 5/12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Employees were not properly trained on how to wear fall protection. This was most recently documented on March 2, 2017 at 1136 W College Street Building 200 Liberty, MO 64068. The employees were wearing fall harness, but were not wearing them properly.
