29 CFR 1926.56(a): General. Construction areas, ramps, runways, corridors, offices, shops, and storage areas shall be lighted to not less than the minimum illumination intensities listed in Table D-3 while any work is in progress: On or about 2/19/2017, the employer did not ensure that the lighting for employees performing work in construction areas met the minimum illumination intensity of 5 foot-candles. Employees performing material salvage work at the Tandem Mill, Bay 103 South End worked in areas where the illumination intensity was measured to be between 0.2 and 2.4 foot-candles, exposing employees to trip and fall hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. On or about 2/19/2017, the employer did not ensure that employees performing work in construction areas were not exposed to trip hazards while performing work activities. Workers were exposed to uncovered floor holes at the Tandem Mill, Bay 103 South End that measured 6 to 8 inches in depth. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about 02/19/2017, the employer failed to report an in-patient hospitalization to OSHA within twenty-four (24) hours. The employer did not notify OSHA that an employee was hospitalized after experiencing a fall of 9 feet in the Tandem Mill, Bay 103 South End work area. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
