29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On or about February 28, 2017, at the jobsite an employee was observed climbing and standing on the guardrails of a scissor lift and using them as a working platform which was not at least 18 inches wide. This exposed the employee to the hazard of falling.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On or about February 28, 2017, at the jobsite an employee was exposed to a fall hazards when observed standing on the mid rail of a scissor lift in order to gain height to perform welding work. The employee was not wearing personal fall arrest and had no other effective means of fall protection.
