McCreary Roofing Company, Inc., 602 West 10th Street, Erie, PA  16502, southeast corner of the structure, on or about March 6, 2017 and times prior thereto:  An employee was exposed to falling/flying roofing debris from roofing activities above.  Employees were throwing roofing materials down to the employee on the balcony.  The employee on the balcony was not wearing head protection.

Employees were exposed to a fall hazard of 10-23 feet to the lower level.  Employees were not utilizing a means or method of fall protection when engaged in the roof activities.

McCreary Roofing Company, Inc., 602 West 10th Street, Erie, PA  16502, south side of the structure, on or about March 6, 2017 and times prior thereto:  Employees were walking/working from the balcony of the structure to perform work activities including, but not limited to, throwing roofing materials into a truck.  The guard railing around the balcony was 34.5-inches in height.  The employees would be exposed to a fall hazard of 13-feet to the ground.
