29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about 03/06/2017 at the Heirloom Flats Construction Project, two employees were using personal fall arrest systems that allowed free falls greater than 6 feet and allowed contact with lower levels. Colt Builders Corp. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1021127, citation number 01, item number 001 and was affirmed as a final order on 09/03/2015, with respect to a workplace located at 2415 Reservoir Avenue in Trumbull, CT.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about 03/06/2017 at the Heirloom Flats Construction Project, employees were not adequately trained on the proper use, operation, and limitations of the personal fall arrest systems in use.
