Armormax Inc. - On or about 3/7/17, and times prior thereto, employees were operating forklifts including, but not limited to, a Clark and Nissan forklifts. A temporary employee had not been trained or evaluated on the forklifts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A.) Armormax, Inc.; shop area - On or about 3/7/17, and at times prior thereto, the Tannewitz bandsaw did not have effective guarding over the saw blade. The guard was not long enough to be adjusted down to the size of the stock being cut and exposed the top of the blade. Additionally, the guard did not cover the side of the saw blade.
