29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): A.) Armormax Refractories, Inc., 3138 Ben Franklin Highway, Edinburg, PA 16116, on or about July 16, 2021 and times prior thereto: Employees were attempting to free a burner block from a preheater lid via a crane/nylon sling and pry bar. During the process, the preheater lid was suspended off the ground. When the burner block freed from the preheater lid, the preheater lid fell onto the employee's left foot resulting in subsequent fractures. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Column K was marked as zero, and did not have the number of days away.
