29 CFR 1926.1412(d)(1): Visual inspections were not performed by a competent person prior to each shift for apparent deficiencies of the equipment that was used: On or about March 8, 2017 and at times prior thereto, the employer did not ensure a tower crane used by employees to move trusses was visually inspected by a competent person prior to use, exposing the employees to struck-by hazards.

29 CFR 1926.1424(a)(2)(ii): 29 CFR 1926.1424(a)(2)(ii): To prevent employees from entering hazard areas, the employer did not erect and maintain control lines, warning lines, railings or similar barriers to mark the boundaries of the hazard areas and/or did not train each employee to understand what these markings signify. On or about March 8, 2017 and at times prior thereto, the employer did not ensure barriers were provided and maintained around the hazardous area of a tower crane to prevent employees from walking and/or working within the path of travel, exposing employees to the hazard of struck-by.
