29 CFR 1910.29(g)(3): Cages and wells are designed, constructed, and maintained to contain employees in the event of a fall, and to direct them to a lower landing; On or about February 15, 2017 an employee climbed the ladder on bin number 13. The cage of the ladder was bent outwards towards the platform to the hatch door. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.29(g)(4): Platforms used with fixed ladders provide a horizontal surface of at least 24 inches by 30 inches (61 cm by 76 cm). On or about February 15, 2017 an employee climbed the ladder on bin 13 to access the platform to open the grain bin hatch door. The employee did not have adequate clearance in order to stand on the platform and, safely open the hatch door to the grain bin. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
