29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about March 3, 2017, at an underground construction site located on the corner of N.E. 50th and Sewell in Oklahoma City, OK, no protective devices were in place to protect an employee working in a trench that was more than 5 feet in depth, exposing the employee to the hazard of being crushed by falling soil.
