29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    a) Front, Left Side of House - Three workers were re-pointing bricks on the outside second level of the structure from an approximate 10 inch wide scaffold platform, 15 feet above the ground, on or about 3/9/2017.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:    a) Left Side Front of House - A 20' aluminum extension ladder was tied to the structure's chimney and suspended down the front of the house to support a platform/plank that had been extended from a scaffolding outrigger, on or about 3/9/2017.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:      a) Front Left Side of Structure -  A 20 foot extension ladder was tied to the chimney of the house,  then suspended down the front of the house and used to support one end of a scaffolding plank, 15 feet above ground, from which the workers were re-pointing bricks and a scaffolding plank at the bottom of the ladder, on or about 3/09/2017.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Front Center of Structure - Workers gained access to the upper levels of the scaffolding by climbing the scaffolding frame and cross braces, on or about 3/9/2017.      Edgar's Masonry Construction Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 571939, citation number 1, item number 2 and was affirmed as a final order on 11/09/2012, with respect to a workplace located at 4538 Cathedral Ave NW Washington DC 20008.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
