29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.    On or about March 10, 2017, the employer did not document that the ammonia refrigeration equipment complies with recognized and generally accepted good engineering practices, such as but not limited to ANSI/IIAR 2-2014, "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigerating Systems":    a) The main door in the engine room communicating with the building was not equipped with panic-type hardware, and did not open outward per 6.10.2.    b) There was not a remote emergency shutdown control immediately outside the designated principle exterior machinery room door, per 6.12.1.    c) There was not an emergency ventilation control switch outside and adjacent to the principle exterior machinery room door, per 6.12.2.    d) The ammonia detectors did not activate a visual alarm outside each entrance to the refrigerating machinery room, per 6.13.1.3.    e) There was no alarm signage adjacent to the visual  and audible alarm devices, per 6.15.2 and 17.6.    f) The emergency eyewash shower outside of the machinery room did not comply with ANSI/ISEA Z358.1, per 6.7.1 and 6.7.3.    g) Pressure relief discharge piping was allowed to terminate less than 7.25 feet above platform surfaces, which did not comply with 15.5.1.4.
