29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: Kasai North America, Inc. - Upper Sandusky, Ohio: On or about March 13, 2017, the employer did not ensure that any forklift operator involved in an accident or near-miss was retrained in topics relevant to safe operations of powered industrial trucks. On March 6, 2017, a forklift operator struck a worker, resulting in a recordable injury, and was permitted to operate a forklift following the accident without refresher training.
