29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached: (a) on the jobsite, atrium - two to three employees were simultaneously attached to each of two Temper Anchor by Guardian Fall Protection butterfly anchors with a breaking strength of 5,000 pounds and a specification for one connection per anchor, exposing employees to a 25 ft. fall hazard from the roof deck; on or about 3/7/2017.
