On or about October 10, 2017 employees were working on a 5:12 pitch roof to install truss blocking and bracing and had utilized a 6 foot, Werner brand fiberglass A frame step ladder to gain access to the top plate of the CMU wall 9 feet 4 inches above the ground. The step ladder was too short for its intended use and the side rails of the ladder did not extend 3 feet above the surface being accessed, nor was it secured to prevent deflection while climbing the ladder.

McKibbin Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR1926.502(d)(15), which was contained in OSHA inspection number 1216653, citation number 1, item number 1, and was affirmed as a final order on August 3, 2017, with respect to a workplace located at 883 E. Lake Road, Tarpon Springs, FL 34688.
