29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   a.  On or about October 10, 2017 employees were working on a 5:12 pitch roof to install truss blocking and bracing and had utilized a 6 foot, Werner brand fiberglass A frame step ladder to gain access to the top plate of the CMU wall 9 feet 4 inches above the ground. The step ladder was too short for its intended use and the side rails of the ladder did not extend 3 feet above the surface being accessed, nor was it secured to prevent deflection while climbing the ladder.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about October 9, 2017 one employee is installing roof sheathing on a 5:12 pitch steep slope commercial roof structure and is not protected by any means of personal fall protection, guardrails or safety nets, exposing them to falls in excess of 10 feet 8 inches, to the ground below.  b. On or about October 10, 2017 three employees are installing roof trusses on a 5:12 pitch steep slope commercial roof structure and are not protected by any means of personal fall protection, guardrails or safety nets, exposing them to falls in excess of 10 feet 8 inches, to the ground below.  McKibbin Enterprises, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR1926.502(d)(15), which was contained in OSHA inspection number 1216653, citation number 1, item number 1, and was affirmed as a final order on August 3, 2017, with respect to a workplace located at 883 E. Lake Road, Tarpon Springs, FL 34688.
