29 CFR 1917.95(b)(1): The employer did not provide and direct the wearing of personal flotation devices for those employees engaged in work in which they may be pulled into the water. F/T Constellation, Crowley Dock, right hand side below attached gangway; on or about 09 March 2017 and times prior thereto; employees were loading ship supplies from the dock's edge through a wall opening onboard the vessel. Employees were hand-passing items over a 32-inch wide opening between the dock and the vessel, with a height of 165-inches above the water. Employees on the dock were not wearing approved personal floatation devices. This condition exposed employees to a fall hazard.
