This exposed employees to a 13 foot fall hazard to the weather deck.    (b)  Weather deck stern port side:  On or about March 3, 2017 and times prior thereto, the fixed ladder had no protection at the top of the ladderway.  This exposed employees to a 13 foot fall hazard to the weather deck.    (a)  Weather deck mid-deck starboard side:  On or about March 3, 2017 and times prior thereto, the fixed ladder had no protection at the top of the ladderway.  This exposed employees to a 9 foot fall hazard to the weather deck.    (d)  Weather deck bowcenter:  On or about March 3, 2017 and times prior thereto, the fixed ladder had no protection at the top of the ladderway.  This exposed employees to a 9 foot fall hazard to the weather deck.    (e)Engine space to boiler section:  On or about March 3, 2017 and times prior thereto, the fixed ladder going up to the boiler had no protection at the top of the ladderway.  This exposed employees to a 8 foot fall hazard.
