29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) Tool Shed, where the outer sheathing on power cord to the Sparton Technology Monitron was damaged and missing portions that exposed insulated conductors.

29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a) Facility, where electrical equipment not used as listed or labeled included:  1. Tool Shed, where the Tripp Lite relocatable power tap was used to supply power to the Wilton 6 inch bench grinder.  2. Annex Building, where the APC relocatable power tap located beneath the Elkay drinking fountain was permanently mounted to the wall and the power cord was run through an unprotected hole in the metal plate on the bottom of the drinking fountain; the APC relocatable power tap used to supply power to audio visual and computer equipment.  The U.S. Customs & Border Protection Border Patrol was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1149914, citation number 2, item number 1 and was affirmed as a final order on July 21, 2016, with respect to a workplace located at Blaine, WA.  The U.S. Customs & Border Protection Border Patrol was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 977497, citation number 2, item number 1 and was affirmed as a final order on July 7, 2014, with respect to a workplace located at Ferndale, WA.  The U.S. Customs & Border Protection Border Patrol was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 242715, citation number 2, item number 1 and was affirmed as a final order on April 4, 2012, with respect to a workplace located at Bellingham, WA.
