29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) At 3748 Airport Blvd., Mobile, Alabama (south side of the building): On or about March 8, 2017, and at times prior; the Controlling Employer Haley Construction, Inc. failed to ensure employees of the sub-contractor,  The World of Masonry, Inc. were using a full body harness and tied off while working from an elevated aerial lift basket exposing the employees to a fall hazard of up to twenty-five (25) feet to the concrete ground below.  b) At 3748 Airport Blvd, Mobile, Alabama (south side of the building): On or about March 8, 2017, and at times prior; the Controlling Employer Haley Construction, Inc., failed to ensure employees of the sub-contractor, Valley Signs and Surveys, LLC were using a full body harness and tied off while working from an elevated aerial lift basket exposing the employees to a fall hazard of up to  thirty (30) feet to the concrete ground below.   THE HALEY CONSTRUCTION, INC.,  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.453(B)(2)(V)), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 983178 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 16, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 1701 SUNRISE HIGHWAY, OLIVE GARDEN SITE, BAY SHORE, NEW YORK AND IN OSHA INSPECTION NUMBER 1002245 CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 03, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 194 ROUTE 17N, ROCHELLE PARK, NEW JERSEY.   .
