29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):        On or about March 16, 2017, the employer did not ensure employees were protected from a fall while performing the following work activities, exposing employees to a fall of over 25 feet.     a)  Employees performing roofing activities on a residential property were exposed to fall while working without fall protection.     b)  Employees walking on the rafters of a  residential property were exposed to fall while working without fall protection.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about March 16, 2017, employees used an "A" frame step ladder that did not extend three feet past the attic they were accessing, exposing the employee to a fall of 10 feet.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.     On or about March 16, 2017, employees used a portable ladder with structural damage, exposing employees to the hazards associated with using a damaged ladder.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
