29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a. On or about March 16, 2017, at the second level warehouse area, employees were exposed to a fall hazard of 10 feet in height to the ground level below, from an unprotected 12 feet open side floor, while loading and unloading packing materials on a Clark propane (LPG) forklift. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
