29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) On or about March 15, 2017 employees installing gas lines on the roof of a commercial building were exposed to falls of approximately 30 feet and were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) On or about March 15, 2017 employees using a scissor lift to transport tools and supplies from the ground to the roof, approximately 30 feet above a concrete surface, were exposed to tripping and falling hazards while stepping over a hole approximately 7.5 inches between the edge of the lift and the edge of the roof.
