29 CFR 1910.110(e)(4)(iii): LP gas container(s) installed on internal combustion engine vehicles were not securely mounted to prevent jarring loose, slipping or rotating: 1) Production Area: On or about March 15, 2017, employees were exposed to an explosion hazard and/or struck-by hazard, in that the holding latch used to secure the LP gas cylinder on the Mitsubishi Hyster #25 forklift truck (model FGC30 and serial AF83B-0031) was missing, leaving the cylinder unsecured. 2) Production Area: On or about March 15, 2017, employees were exposed to an explosion hazard and/or struck-by hazard, in that the holding latch used to secure the LP gas cylinder on the Mitsubishi Hyster #20 forklift truck (model FGC30) was missing, leaving the cylinder unsecured.
