29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. On or about March 17, 2017, at a residential home construction site, an employee using a pneumatic type nailer to nail install sheathing materials was not using eye protection and was exposed to eye injuries caused by airborne debris generated by the nailing process.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about March 17, 2017, at a residential home construction site, on a steep roof with a 5:12 pitch, five (5) employees who were removing the existing roof were not protected by any form of fall protection while exposed to a eight (8)-foot, two (2)-inch fall hazard to the ground below.
