29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. At a single story, commercial building used as a golf pro shop and restaurant, five (5) employees using pneumatic type nail guns to install new roofing shingles are not using eye protection and are exposed to eye injuries caused by airborne debris generated by the nailing process: The employer fails to assure effective protection against eye injuries as required by the standard, as demonstrated on September 7, 2017, when five (5) employees installing roofing shingles on the roof of a commercial building located at 659 Leonard C. Taylor Parkway, Green Cove Springs, Florida 32043, are not protected from eye injuries hazards by the use of a safety eye protection.  Quality Discount Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1218037, citation number 1, item number 1 and was affirmed as a final order on 04/25/2017, with respect to a workplace located at 12167 Silver Saddle Drive in Jacksonville, Florida 32207.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. At a single story, commercial building used as a golf pro shop and restaurant, employees working at elevations of six feet or more above the lower level are not being protected from fall hazards using fall protection systems: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on September 7, 2017, when five employees installing roofing shingles on the roof of a commercial building located at 659 Leonard C. Taylor Parkway, Green Cove Springs, Florida 32043, are not protected from a  eleven (11)-foot, four (4)-inch fall hazard by the use of a personal fall protection systems.  Quality Discount Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1223675, citation number 1, item number 1 and was affirmed as a final order on 05/16/2017, with respect to a workplace located at  9409 War Hawk Road, Lot 5, Jacksonville, Florida 32210.  Quality Discount Roofing, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1218037, citation number 1, item number 2 and was affirmed as a final order on 04/25/2017, with respect to a workplace located at 12167 Silver Saddle Drive in Jacksonville, Florida 32207.
