29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: (a) Marriott Fairfield Inn & Suites, Fourth floor rear side: On or about March 16, 2017 , and at times prior, the employer exposed employees to fall hazards of more than 30-feet from a job-made work platform attached to a JLG G10-55A Telehandler, in that the man-lift platform used by the window installers was not an approved work platform by the manufacturer.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) Marriott Fairfield Inn & Suites, Fourth floor rear side: On or about March 16, 2017, and at times prior, the employer exposed employees to fall hazards of over 30-feet to the lower level while working from a job-made man-lift platform, in that the platform used by the window installers was not secured to the forks of the JLG G10-55A Telehandler Forklift Truck.
