29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer did not provide abatement documentation proving that the hazards related to the PPE, Fire Extinguishers, and Electrical Safe Work Practices citations that were issued on June 16, 2017.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about March 20, 2017, the employer was unable to verify that a workplace hazard assessment had been performed for employees engaged in job tasks such as welding, grinding, and torch cutting.  MODS International Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(2) which was contained in OSHA inspection number 1169698, citation number 2, item number 1 and was affirmed as a final order on December 8, 2016, with respect to a workplace located at 5523 W. Integrity Way Appleton, WI 54913.

29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.  On or about March 20, 2017, portable fire extinguishers mounted in the shop were not provided an annual maintenance check and the employer was unable to provide a record upon request.  MODS International Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3), which was contained in OSHA inspection number 1169698, citation number 1, item number 2 and was affirmed as a final order on December 8, 2016, with respect to a workplace located at 5523 W. Integrity Way Appleton, WI 54913.

On or about March 20, 2017, the employer did not ensure that powered industrial truck operators were trained and evaluated to operate the JCB Model 512-56 rough terrain and Clark Model CCS15 LP powered forklifts. 29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about March 24, 2017, flexible cord sets were not visually inspected before use as a 240 volt extension cord with damaged sheathing was placed into service to power welding equipment.
