29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:   (1) On or about March 21st, 2017, at a construction site located at 2001 Main Street in Kansas City, Missouri, an employee was observed working on top of a rooftop utility building more than 6 feet above the lower rooftop area.  Although the employee was wearing a body harness and lanyard connected to a lifeline, the system was rigged such that the employee could have fallen more than 6 feet to that lower rooftop level.
