29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    Ross Enterprises, 770 Peru Olena Rd. Norwalk, OH, on or about March 21, 2017 the employer did not ensure that employees operating a fork truck to move material on the work site were adequately trained. The employee had not completed a combination of formal instruction and practical training  as well as an evaluation of their ability to perform in the workplace.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section.    Ross Enterprises, 770 Peru Olena Rd. Noralwalk, OH, on or about March 21, 2017, employees were exposed to a fall hazard while descending 12 feet from a roof to the ground via a front end loader. Employees were standing on top of material being lowered to the ground.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Ross Enterprises, 770 Peru Olena Rd. Norwalk, OH, on our about March 21, 2017, the employer did not ensure that employees were protected from falls while performing tasks associated with roofing. Employees performing roofing operations on a residential roof were exposed to fall hazards of approximately 12 feet on the unprotected edges of the roof.
