Salenbien Trucking and Excavating, Inc. worksite located on Lincoln St., Delta, Ohio: On or about March 21, 2017, the employer did not ensure employees were adequately protected from cave-ins while installing a 24 inch sanitary sewer line approximately at a depth of approximately 17.00 feet. Employees were exposed to approximately 7.00 feet of material potentially collapsing above the trench box.     Salenbien Trucking and Excavating, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.652(a)(1), which was contained in OSHA inspection number 1172694, citation number 1, item number 1, and was affirmed as a final order on October 4, 2016 with respect to a workplace located at North Baltimore, Ohio.
