29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more methods listed in this section: (a) On and before March 22, 2017, at the Stacker Lines #1 and #2, employees were exposed to fall hazards up approx. 7 feet during times of access and walking on the upper surfaces for re-positioning and un-bunching activities of the leather products on conveyor type machinery. The employer had not provided the appropriate Fall Protection Equipment or Fall Protection System for employees to safely access the aforementioned areas. As a result, employees could not and did not wear the inadequate equipment that was provided when exposed to the fall dangers of the areas.
