29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a)  On or about March 1, 2017, and times prior to that date, employees were exposed to struck by hazards created by utilizing a forklift boom manufactured by West Side Fabrication, LLC attached to a Toyota Fork Lift Truck model # 2FPCA25, serial # 2FPCA25-12223 that was not approved for use by the manufacturer.
