29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) Commercial jobsite: On March 23, 2017 and at times prior thereto employees performed roofing activities on a low sloped roof of a commercial building. No fall protection system was in place along the side walls or along the open back, exposing employees to a fall hazard up to approximately 32 feet to the street below. Note) Certification of corrective action is required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) Commercial jobsite: On March 23, 2017 and at times prior thereto an extension ladder was used for access to the roof area. The ladder did not extend at least 3 feet above the side wall nor was it secured to the building. Note) Certification of corrective action is required for this item.
