29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: At the location of the bagger on the feed side the box attached to the drive motor that enclosed the electrical wiring had been knocked loose and was hanging by the electrical wires. Employees were exposed to a shock hazard.

29 CFR 1910.28(b)(3)(iv): The employer did not ensure that each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. Employees utilizing the ladderway above Round Bin Number 12 are not protected from falling by a self-closing gate or an offset. Employees are exposed to a fall hazard of approximately fifteen (15) to the square bin below. Western Feed Mills Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(a)(2), which was contained in OSHA inspection number 896963, Citation number 01, Item number 01 and was affirmed as a final order on 06/03/13, with respect to a workplace located at 401 S Sale Barn Rd, Cedar Vale, Kansas.
